Systolic murmur is a murmur heard while the heart is contracting. Basically meaning there is an inefficiency in emptying the heart and sending blood around the body. He had a leaking heart valve.

Written in his notes:  He died from the kidney mischief. I believe that was there all along but was over looked.

The presence of murmurs is an unfavourable element, as the heart has a very hard task to perform to compensate for the renal defect, and if handicapped by a valvular insufficiency it will probably fail early in the struggle. From the point of view of the prognosis of heart disease, the supervention of kidney mischief is very unfavourable. A valvular defect, which was apparently compensated, will acquire fresh importance, and be followed by rapid heart failure and death.
